Saudi Arabia's King Salman has ordered a safety review for the Hajj pilgrimage after at least 717 people died in a stampede near the holy city of Mecca.
Another 863 people were injured in the incident at Mina, which occurred as two million pilgrims were taking part in the Hajj's last major rite.
It is the deadliest incident to occur during the pilgrimage in 25 years.
The king said there was a need "to improve the level of organisation and management of movement" of pilgrims.
A commission to investigate the crush has been formed by the Saudi government
The Saudi health minister, Khaled al-Falih, said the crush occurred because many pilgrims moved "without respecting the timetables" established by authorities
Ayatollah Ali Khamenei, the supreme leader of Iran, which lost at least 95 of its citizens in the crush, said the Saudi government "must accept the huge responsibility for this catastrophe"
He added that "mismanagement and improper actions" were to blame
It is the second disaster to strike Mecca in two weeks, after a crane collapsed at the Grand Mosque, killing 109 people.

Considering the number of people making the journey, I am surprised there isn't a lot more of this. Densely packed crowds can easily spell disaster.

I have twice been in crowds where the same thing could happen (but thankfully did not). It's scary as hell because there's nothing you can do once the corwd surges a certain way.

It seems to me that almost every year something like this happens in Mecca...
You would think the Saudis would start implementing some modern crowd control and crowd flow methods to reduce the likelihood of these tragedies...

Amen to that. I was at the Nassau Colesium (RIP) in '75 (maybe '76) trying to get tickets to an Elton John concert. I got pinned up against the glass on the outside ot the arena. These panels ar maybe 20 feet tall and I kept waiting for them to break and me to be cut to ribbons. No way to push back against the surge, no way to get away. I wasn't suffocating, just waiting for the glass to break. FWIW "the" cause of the tragedy was the failure by police to direct the crowds to the open doors to the terraces right and left of the central pen which was already disastrously overcrowded. That remedy had been applied before and trouble had always been avoided - as had delaying the game start so that incoming people didn't get anxious to push ahead to avoid missing the action. Unfortunately, it was a new cop in charge - the experienced guy had moved on - and the idiot ignored all sensible policing suggestions from his own team. It's true that because of the crush outside the main gates, police opened two more gates to relieve pressure - but they expected the interior guidance of their colleagues to ensure the crowd did not all try to pack into the one pen door.
