I copied the following from a statement I received....

Amount Provider Billed the Plan $425.00

Total cost (amount the plan approved) $10.56

Plan's share $10.36

Your share $0.00

There are four separate blood tests billed, totaling $1336.00 and the total amount paid by the plan is $37.46 with zero cost to me.
