Surely Dame Mirren knows better than to engage in such a shocking breech of etiquette...
I know this is a long shot, but perhaps The Daily Mail took a camera shot out of context just to make it look like that's what she's doing...

The denial of treatment to oldsters is a natural result of nationalised medical treatment. It is not inevitable - but it is natural. Sweden for example has a very pragmatic view of treating heart disease - or prostate cancer - in older men.

If Helen had actually been lecturing and angrily wagging her finger at The Queen, (as was clearly what the Daily Mail was trying to imply with this photo) Dave wouldn't have been looking on bemused, (He'd have been looking uncomfortably in some other direction) and Her Majesty wouldn't be smiling...(as she is in the photo)
Instead she would have been regarding Dame Mirren with a piercing stone-faced glare...
(She's good at that...)
