Without going into detail, the Pennsylvania inspection system is pretty good. the mechanic is required to be trained, he is checking for a set list of safety issues, and if he is competent, the process can very well save a life or two.
Humans abuse things, and mechanics can be unethical, but the system we have is well-intentioned and worth the money we spend to keep our cars - at least arguably - safe.

In PA there are mandatory inspections for cars and trucks, motorcycles and trailers over 3000lb or with brakes.
Pa is also the toughest state to pass a 'safety inspection' so those states with an annual, simply base it off our book.
We also have varying levels of emission inspection.

Theoretically, my state has inspections. Of course, I was told a while back that passing would cost me $100. I was also told flatly that without $100, the truck would not pass. He didn't actually need to see the truck to pass it, just the cash.
