I believe this particular death was an example of what is sometimes called the paradox of the commons. When responsibility is shared by everyone, then no one is. It is taken for granted that somebody else is taking care of it. This was an extended family gathering. It would have been very easy for the parents to accept that all the grown ups in this gathering were good people and 'somebody' would just naturally be watching this small boy at any one time and would shout a warning if there was a problem.

But 'someone' just did not act fast enough. However, this surely seems to me to be a foreseeable event on the part of the designers and operators of this restaurant. Not so much so on the part of parents. How much experience did the parents have in child-proofing revolving restaurants?
