Actually, it's quite a serious issue; while great strides have been made, there are still hundreds of millions of people worldwide who defecate in the open. It's a major cause of waterborne illness (cholera, typhoid, etc.), and every $1 spent on improving sanitation returns $4 in reduced medical costs.

IMHO, the two things that have contributed most to the progress of civilization are sanitation followed by vaccines; but I guess Robert Kennedy Jr will take on sanitation projects next. Ignorance is the biggest threat to civilization, followed by arrogance/hubris, and Trump et al. have taken pains to provide both.
